  • Who is my named GP?

    All Patients haven been allocated a named GP who is responsible for the Patient's overall care.

    Please contact the surgery if you would like to know the name of your GP.

In accordance with the Data Protection Act 1998 and GDPR 2018 patients may request to see all or part of their medical records. Requests can be made in writing, or verbally to the practice. There is no charge for this unless requests are excessive.

Through the online services, you can now view your summary care record or access your detailed coded medical record. To access this facility you will need to complete an application form and provide photo identification (Passport or Driving Licence) and proof of residency (utility bill or bank statement). Your GP will review your record and has the right to redact information relating to 3rd parties or sensitive information which may cause harm.

In accordance with Access to Healthcare Records Act 1990 the practice will provide copies of your medical record to 3rd parties for purposes such as setting up insurance, police reports, and solicitors reports. No information will be released without the patients consent unless we are legally obliged to do so. There is an administration fee for these requests. 



The surgery has three reserved car parking spaces for disabled patients, all located at the front of the building. We have suitable access to the surgery, our consulting rooms, treatment areas and toilet facilities. A lift is available to access our upstairs consulting rooms.



The practice is a dispensing practice. Patients who live outside Boroughbridge and its immediate area are entitled to have their prescriptions dispensed from us at the surgery. Those who live within the immediate area of the practice and within a mile of the nearest chemist must use the local pharmacy.



Church Lane Surgery is committed to providing a safe environment where patients and staff can be confident that best practice is being followed at all times. 

The practice has a Chaperone policy in place and a copy is available on request from the Practice Manager.



The practice will accept applications from patients, or proxy’s for online services. Identity verification is required before online service applications can be processed. The verification checks are used to establish that the identity of applicant for online services matches that of the individual registering with or registered at the practice and recorded on the GP system.

At the point of application the practice will verify identity using one of the methods detailed below:

Documentation: – One form of photo ID and one proof of address, which will be photocopied:

  • Either of the following: Passport or Photo Driving Licence


  • one official letter bearing your name and address dated within the last three months from any of the following: your bank, building society, local council, utility company or landline telephone provider

(Note: a photographic driving licence will suffice for both photo ID AND proof of address)  

The practice requires 20 working days to process enhanced access (access to detailed coded medical records) to allow for the GP data checks to be completed.

Note: The practice requires that the patient / proxy provide proof of identity on collection of user credentials. User credentials will not be released without this.

All GP practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients at each practice.

The average pay for GPs working at Church Lane Surgery in the last financial year (2017/2018) was £65,700.20 before tax and National Insurance.  This is for 3 full-time GPs and 8 part-time GPs who have worked in the practice for more than six months.

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the 'classes' of information the practice intends to routinely make available. This scheme is available from reception.



The practice is computerised. Patients who register with the practice will have their personal and clinical details recorded on the computer system. All new patients will be asked to complete a simple questionnaire about their health, past medical history, family history, address and telephone details. We ask that patients keep the practice informed of any change in these details as they occur.

  • Travel Abroad (e.g. Vaccination/Holliday cancellation/Fitness to travel certificates)
  • Access to records under the Data Protection Act
  • Certificates and Forms (e.g. Shotgun certifcate, Private Medical Insurance claim form, Sickness/accident insurance benefit claim form)

We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded on the computer and we are registered under The Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of the multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the practice team.

New contractual requirements came into force from 1 April 2014 requiring that GP Practices should make available a “statement of intent” in relation to the following IT developments:

  • Summary Care Record (SCR)
  • GP to GP Record Transfers (GP2GP)
  • Patient Online Access to their GP medical record

The same contractual obligations require that we have a “statement of intent” regarding these developments in place and publicised by 30 September 2014.

Our practice upholds a strict policy towards violent or abusive behaviour on our surgery premises. Any person involved in an act of violence or threatening behaviour towards any member of our practice staff, or anyone else on our premises, will be removed from the practice list.
