    Please ensure you have enough medication to carry you through the holiday period and please order in plenty of time, thank you.

  • Upcoming Surgery Closure

    Please note the surgery will be closed for a Training Afternoon on: 

    Thursday 8th February 2018 from 1pm.

    The surgery will reopen as usual the next day, Friday 9th February 2018 from 8am.

  • Dispensary Opening Times

    Please be aware the Dispensary is not open on a Saturday morning when the Surgery is running a Saturday morning clinic.

    The Dispensary hours are Monday - Friday, 8.30am - 6pm.

     

     
  • Who is my named GP?

    All Patients haven been allocated a named GP who is responsible for the Patient's overall care.

    Please contact the surgery if you would like to know the name of your GP.

    Although you have a named GP, you can still make appointments with any of our doctors.

     If you wish you can also ask to change your named GP and we would aim to accommodate this request wherever possible.

     
  • Online services for 11-16 year olds

    Unfortunately for operational reasons, online services will not be available to patients aged between 11-16 years.

    TPP have released a SystmOnline app, available currently for the IOS platform, which offers patients much of the same functionality as the SystmOnline website, but optimised for smartphones.

ACCESS TO MEDICAL RECORDS

All patients' medical records (both paper and computerised) are confidential. However, there are times when it is necessary for some patient records to be looked at by medical personnel from outside the practice in order to assess the standard of clinical care being given within the practice.

You may now view your summary care record and access your detailed coded data with your medical record through the online services. A completed consent form along with Photo identification and proof of residency will be required to access this facility.

 

DISABLED ACCESS

The surgery has three reserved car parking spaces for disabled patients, all located at the front of the building. We have suitable access to the surgery, our consulting rooms, treatment areas and toilet facilities. A lift is available to access our upstairs consulting rooms.

 

DISPENSING

The practice is a dispensing practice. Patients who live outside Boroughbridge and its immediate area are entitled to have their prescriptions dispensed from us at the surgery. Those who live within the immediate area of the practice and within a mile of the nearest chemist must use the local pharmacy.

CONSENT FOR ONLINE SERVICES

Application for Online Services
The practice will accept applications from patients or proxy’s for online services. The application form can be obtained from the surgery when the patient initially registers with the practice or at any time while they are registered with the practice or downloaded from our website - please click on the relevent link below:

Consent form for Parental/Guardian Proxy Access (under 11 years)

Identity verification is required before online service applications can be processed. The verification checks are used to establish that the identity of applicant for online services matches that of the individual registering with or registered at the practice and recorded on the GP system.

At the point of application the practice will verify identity using one of the methods detailed below:

Documentation – one form of photo ID and one proof of address, which will be photocopied:

  • Either of the following: Passport or Photo Driving Licence

AND

  • one official letter bearing your name and address dated within the last three months from any of the following: your bank, building society, local council, utility company or landline telephone provider

(Note: a photographic driving licence will suffice for both photo ID AND proof of address)  

The practice requires 20 working days to process enhanced access (access to detailed coded medical records) to allow for the GP data checks to be completed and 5 working days for the standard access ( access to appointments, repeat prescriptions and summary care record).

Note: The practice requires that the patient / proxy provide proof of identity on collection of user credentials. User credentials will not be released without this.

 

 

COMPLAINTS

If you have a complaint about the service you have received from the doctors or any of the staff working in this practice, please let us know.  We operate a practice complaints procedure as part of the NHS system for dealing with complaints.  Please click on the link to read through the procedure.  Our complaints system meets the national criteria.

 

GP NET EARNINGS

All GP practices are required to declare the mean earnings (e.g. average pay) for GPs working to deliver NHS services to patients at each practice.

The average pay for GPs working at Church Lane Surgery in the last financial year, was £66,150 before tax and National Insurance.  This is for 2 full-time GPs and 9 part-time GPs who have worked in the practice for more than six months.

FREEDOM OF INFORMATION ACT – PUBLICATION SCHEME

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the 'classes' of information the practice intends to routinely make available. This scheme is available from reception.

 

NEW PATIENT REGISTRATIONS

The practice is computerised. Patients who register with the practice will have their personal and clinical details recorded on the computer system. All new patients will be asked to complete a simple questionnaire about their health, past medical history, family history, address and telephone details. We ask that patients keep the practice informed of any change in these details as they occur.

All patients will be allocated a named GP who is responsible for the Patient's overall care. Although you will have a named GP, you can still make appointments with any of our doctors. If you wish you can also ask to change your named GP and we aim to accommodate this request wherever possible.

 

NON NHS SERVICES

Church Lane Surgery is pleased to provide additional medical services that are not provided by the NHS such as:

  • Insurance Company Reports
  • Medical Examination & Reports (e.g. HGV/Taxi Driver medicals, Private blood test, Pre-employement) 
  • Letters
  • Travel Abroad (e.g. Vaccination/Holliday cancellation/Fitness to travel certificates)
  • Access to records under the Data Protection Act
  • Certificates and Forms (e.g. Shotgun certifcate, Private Medical Insurance claim form, Sickness/accident insurance benefit claim form)

PATIENT CONFIDENTIALITY AND DATA PROTECTION

We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded on the computer and we are registered under The Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of the multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the practice team.

STATEMENT OF INTENT FOR CHURCH LANE SURGERY

New contractual requirements came into force from 1 April 2014 requiring that GP Practices should make available a “statement of intent” in relation to the following IT developments:

  • Summary Care Record (SCR)
  • GP to GP Record Transfers (GP2GP)
  • Patient Online Access to their GP medical record

The same contractual obligations require that we have a “statement of intent” regarding these developments in place and publicised by 30 September 2014.

VIOLENT PATIENTS

Our practice upholds a strict policy towards violent or abusive behaviour on our surgery premises. Any person involved in an act of violence or threatening behaviour towards any member of our practice staff, or anyone else on our premises, will be removed from the practice list.
