  • CHANGES TO THE REPEAT PRESCRIPTION TELEPHONE SERVICE 

    From Monday 12th November 2018 access to the repeat prescription telephone service will be available from:

    12.30pm to 3pm

    Monday - Friday

    You can still order your repeat medications in person, on-line, through the App or via the post.

    Please be aware the Dispensary is not open on a Saturday morning when the Surgery is running a Saturday morning clinic.

    The Dispensary hours are Monday - Friday, 8.30am - 6pm.

     

     
  • Who is my named GP?

    All Patients haven been allocated a named GP who is responsible for the Patient's overall care.

    Please contact the surgery if you would like to know the name of your GP.

    Although you have a named GP, you can still make appointments with any of our doctors.

     If you wish you can also ask to change your named GP and we would aim to accommodate this request wherever possible.

    Unfortunately for operational reasons, online services will not be available to patients aged between 11-16 years.

    TPP have released a SystmOnline app, available currently for the IOS platform, which offers patients much of the same functionality as the SystmOnline website, but optimised for smartphones.

Make an appointment

To make an appointment at either the main or branch surgery:

  • Please telephone, Monday to Friday:

The Main surgery on (01423) 322309 between 8.00am and 6.00pm
or
The Branch surgery (01423) 360296 between 8.30am and 12.30pm

or

  • call into the surgery in person.

 

If possible, it would help other patients if you could arrange an appointment during the day and leave the early morning / evening appointments for those who are not able to attend the surgery during the day.

 

If you URGENTLY need to see a doctor, please telephone the surgery as early as possible in the day.  The Surgery is opened from 8.00 am Monday to Friday.  All URGENT cases will be seen on the same day, but perhaps not by the doctor of your choice.

 

Additional Extended Access

Church Lane Surgery is now offering additional extended access on alternate Saturday mornings in association with the Yorkshire Health Network.

The service runs between 8am and 12pm.  

We are providing additional doctor and practice nurse appointments for our patients plus pharmacist review appointments. 

This is an opportunity for us to be able to offer blood testing appointments on a Saturday morning which will be a new service.    Like our current Saturday surgeries these appointments must be booked in advance as there will be no phone access on the day.

Please note the Dispensary is not open on a Saturday morning. The Dispensary hours are Monday – Friday; 8.30am - 6pm. 

 

Appointments with the Nursing Team

The Nursing Team are available on an appointment basis only:

Monday - Friday 8.00am to 5.30pm

and alternating weeks; Fridays from 7am and Saturdays from 8.00am.

The practice has a computerised appointments system

Doctors appointments made through the surgery can be made up to a maximum of 6 months in advance and online up to 4 weeks.  Please try to make routine appointments for the doctor of your choice well in advance.  Separate appointments will be required for each person who needs to see the doctor.

Nurse appointments can be made a couple of months in advance through the surgery but unfortunately cannot be made through the online booking system.

 

Cancelling your Appointment

If you are not able to attend an appointment with one of the doctors or nurses, it is important to advise us as soon as you know you are unable to attend.

By giving us as much notice as you can you are helping us make available your appointment to another patient.
You can cancel your appointment in person, via the telephone or by going online
